James A. Baldock, MD
Dr. Baldock is a graduate of the University of Kentucky. He attended the University of Kentucky Medical School and completed his residency at the Medical University of South Carolina. He is board certified by The American Board of Urology and is a Fellow of The American College of Surgeons. He served as Chief of Urology at Baptist Medical Center from 1993-2000. He joined the McIver Clinic in 1982.

Charles G. Cobb, MD

Dr. Cobb did his undergraduate studies at Tufts University, graduating with a BS degree in Chemistry, summa cum laude. He completed his medical studies at Vanderbilt University School of Medicine, and completed a urology residency at the University of Texas, Southwestern Medical Center (Dallas, TX). His primary practice is located at McIver’s Baptist Medical Center South location, where he serves as Chairman of Surgery.

Paul M. Crum, MD
Co-Director, Jacksonville Prostate Center
Dr. Crum is a graduate of the United States Naval Academy. He attended medical school at the University of Pennsylvania and completed his residency in Urology at the Naval Hospital (Philadelphia, PA). He served as Chief of Urology at two Naval Hospitals (Portsmouth, VA. and Jacksonville, FL), and at St. Vincent's Medical Center, Jacksonville, FL. He has also served as the Chief of the Department of Surgery, St. Vincent's Hospital (Jacksonville, FL). Currently, he is on the Cancer Committee at St. Vincent's. Dr. Crum is certified by the American Board of Urology and a diplomate of the American College of Surgery.

David L. Dalton, MD

Dr. Dalton is a graduate of Vanderbilt University and University of Tennessee College of Medicine. He was trained in urology and completed his residency at Duke University. He has served as Chief of Urology at St. Luke's Hospital and Baptist Medical Center Downtown, and has been of the Board on the Florida Urologic Society. Dr Dalton is board certified by The American Board of Urology and a Fellow of The American College of Surgeons.

C. Mark Homra, MD
Director, Sexual Medicine & Prosthetic Implant Program
A native of Kentucky, Dr. Homra obtained his undergraduate degree from Vanderbilt University, and graduated from the University of Louisville Medical School and completed his Urology residency. He has lived in Jacksonville since 1997, and currently is Chief of Urology and Chairman of the Credentials Committee at St. Luke's Hospital. He is board certified by the American Board of Urology.

Richard H. Lewis, MD

Dr. Lewis is a graduate of Guilford College and Duke University School of Medicine. He completed his residency at National Naval Medical Center in Bethesda. He is Board Certified by the American Board of Urology and is a Fellow of The American College of Surgeons. Dr. Lewis is currently the Chief of Urology at St. Vincents Hospital.

Charles H. Pruden IV, MD

Dr. Pruden graduated from Wake Forest University and Bowman Gray School of Medicine, where he was a member of the medical honor society. He continued his education at the University of Florida in Gainesville where he completed training in urology. He serves on the credentialing committee at St. Vincent's Hospital.

Sagar R. Shah, MD
Director, Jacksonville Continence Center
Dr. Shah completed his undergraduate degree at the University of Florida and attended the University of Miami School of Medicine where he was elected to the Alpha Omega Alpha Society prior to graduation. He completed a residency in Urologic Surgery at the Medical College of Georgia followed by a two-year fellowship at New York University in Female Pelvic Medicine and Reconstructive Surgery and obtained dual-board accreditation by the American Board of Urology and American Board of Obstetrics and Gynecology. To date, there are only seven similarly accredited fellowships for urologists in the country.

Douglas A. Swartz, MD
Director, Urologic Oncology Program

Dr. Swartz is a graduate of the University of Texas Medical School. His completed his residency at the University of Texas (Urologic Surgery Program at Hermann), St. Joseph’s, and MD Anderson hospitals (Houston, TX). He is board certified by the American Board of Urology. Dr. Swartz has taught for the University of Florida Department of Surgery (Jacksonville) from 1985 to 1997, and was chief of Urology from 1987 to 1997.
He has published 28 peer-reviewed articles and abstracts in urology, and is involved in many active research studies.

Apoorva R. Vashi, MD
Director, Robotic Prostate Program
Dr. Vashi completed his undergraduate, medical, and urology training at the University of Michigan. He joined McIver Clinic in 1998, and has served on various local and regional committees with the Florida Urological Society, American Cancer Society and Baptist Medical Center. Dr. Vashi has been a surgical proctor for Intuitive Surgical (makers of the da Vinci© Robot) since 2006. In 2010, he was chosen by SSI Innovations as one of four US urologists to bring robotic surgery to Southeast Asia. The International Centre for Robotic Surgery (ICRS) will open its first location in Delhi, India in 2011.

C. Shawn West, MD

A native Floridian, Dr. West is a graduate of Florida State University. He attended University of Florida College of Medicine where he completed his urology residency. Following his urologic training, Dr. West joined McIver Clinic in 2008. He is a laparoscopic surgical proctor for PRI Medical and Covidien Medical and is President of Lithotripsy Services of Greater Jacksonville. Dr. West is board certified by The American Board of Urology.
